[Utility of ST/HR slope and modified ST/HR score in the prediction of coronary artery disease in standard treadmill exercise test].

概要 Elaminら の 提唱 したST/HR slopeを 標 準多 段階 運動 負荷試 験 に応 用 し,冠 動脈 疾 患 (CAD)の 有 無 とその重症 度診 断 にお ける有用性 を他 の診断基 準 と比 較検討 した.さ らにこれ を 改良 した修 正ST/HR score(MSHS)を 考案 し,そ の診断 的価値 を検 討 した.冠 動脈 造影施 行 79例 で,運 動 負荷時 のaVFとV5誘 導のST indexとST/HR slopeを 算 出 した. CAD患 者 の診 断 率 は,通 常のST判 定基準 とST indexで は57%と71%で あ り, ST/HR slopeで も72%で あった. ST/HR slopeで の みCAD中 の3枝 病変 の診断 が可能 であ った. MSHSで はCAD診 断率 は96% と良 好 で,CAD重 症 度 判定 も可能 であ った. ST/HR slopeは3枝 病変 の診 断 に,そ れ を改 良 し たMSHSはCAI)の 有 無 と重症 度診 断 に有 用 であ る と思 わ れた.
