The suggestion that D2 receptors are present in the gastrointestinal tract is based on firm pharmacological data from animals; dopamine produces gastric stasis which can be inhibited by specific D2 antagonists, but not by prazosin, a specific a receptor antagonist.4 Similarly, dopamine produces gastric stasis in man.5 The ability of the specific D2 receptor antagonist domperidone to antagonise this argues in favour of a similar mechanism in man. We therefore do not accept that our claims are unsubstantiated; they are based on a reasoned assessment of available clinical and scientific data. We feel that the data presented by Drs Leeser and Bateman are derived from a small preliminary study. We would welcome further debate when they have considered their own findings more fully in the context of the information available in international published reports on the use of domperidone in Parkinson's disease.
