Hypoxic conditions may play a role in pathogenesis of PD. Current investigations connect increased levels of matrix-degrading metalloproteins after hypoxic conditions with transepithelial channel formation by loosening interkeratinocyte bonds, disruption of the basement membrane, and detaching of collagen fibers.4 Kawakami et al.6 proposed that PD may arise from the combined trauma of scratching and dermal necrosis resulting from a poor blood supply associated with diabetic vasculopathy. We speculate that inhibition of VEGF may cause hypoxia that contributes to develop transepidermal elimination of dermal collagen. To our knowledge, this is the first report of a possible correlation between bevacizumab and perforating disorders and may initiate further investigation on similar observation.
