A 28-Year-Old Female with a Right Perihilar Mass

A 28-year-old female with intermittent chest pain and dyspnea was seen in consultation for a right perihilar mass detected on a chest radiograph. She denied cough, wheezing, hemoptysis, fever, night sweats, weight loss, or extremity edema. She had not traveled recently outside Southern California where she was stationed with the military. Relevant past medical history included surgical correction of an omphalocele in infancy, a one-quarter pack-per-day smoking habit, and occasional alcohol use. Her family history revealed that both grandmothers had died of stroke. On examination, her vital signs were normal, including oxygen saturation measured by pulse oximetry in the sitting and supine positions. Cardiac auscultation revealed a regular rhythm, normal heart tones, and no murmur. Digital clubbing was not present.
