Peripheral neuropathy in rheumatoid artritis: is it a result of rheumatoid vasculitis or is it a complication of leflunomide treatment?

Vaskülit RA seyrinde görülen komplikasyonlardan biri olmas›na ra¤men son y›llarda hastal›¤› modifiye eden anti-romatizmal ilaçlar›n (DMARD) ve biyolojik ajanlar›n artan kullan›m› ile oldukça nadir görülmektedir. Romatoid artritte mononöritis multipleks ve distal sensörimotor nöropati vaskülitle birlikte görülebilir. Tan› sural sinir biyopsisinde vaskülit bulgular›n›n saptanmas› ile koyulur. Leflunomid, 1998’den beri kullan›mda olan bir DMARD’d›r. ‹soksazol türevi bir ön ilaç olup aktif metaboliti A77 1726’n›n immunsupresif özelli¤i vard›r, inflamasyonu bask›lar, k›k›rdak ve kemik hasar›n› önler. ‹yi tolere edilen bir ilaç olmas›na ra¤men baz› yan etkileri de vard›r.
