Modified Split Ulnar Gutter Splint for Treatment of Fifth Metacarpal Neck Fractures

흔히 권투 선수 골절(boxer’s fracture)이라고 불리는 제5중 수골 경부 골절은 전체 수부 골절의 20%를 차지할 정도로 가 장 흔한 수부골 손상이다. 이 골절은 주먹을 쥔 상태에서 물체 를 가격할 때 주로 발생하며, 제5중수골은 형태상 다른 중수골 에 비해 가늘고, 위치상 주위조직의 보호가 약하여 비교적 쉽 게 골절이 발생하는 것으로 알려져 있다. Ford 등은 골절부 각 형성이 70�까지도 보존적 치료가 가 능하고 치료 후 기능에 있어 정상과 차이가 없다고 하였고, Theeuwen 등은 수술적 방법으로 후방 각 형성을 교정하는 것이 기능에 큰 이득이 없다고 하여 보존적 치료가 시행되어 왔다. 하지만 최근 발표된 실험 결과에서 골절부 각형성이 30�이상이 되면 관절을 가로지르는 근육, 건의 작용 효율에 영향을 주어 중수지간 관절 굴곡 기능에 장애가 있음이 밝혀 졌고 외관상으로도 골절부 각형성 교정이 안 되었을 경우 주 먹을 쥐었을 때 중수관절 배부(knuckle part)의 함몰이 발생 할 수 있어 부적절한 보존적 치료로는 만족할 만한 치료 결과 Modified Split Ulnar Gutter Splint for Treatment of Fifth Metacarpal Neck Fractures
